Topical NSAIDs May Help Older Patients With Knee Osteoarthritis " "If topical NSAIDs are as effective as oral NSAIDs for reducing knee pain but produce fewer adverse effects, then topical treatment might be preferred," …"

Hylan versus hyaluronic acid for osteoarthritis of the knee: A systematic review and meta-analysis. (Arthritis Rheum. 2007) "CONCLUSION: Given the likely lack of a superior effectiveness of hylan over hyaluronic acids and the increased risk of local adverse events associated with hylan, we discourage the use of intraarticular hylan in patients with knee osteoarthritis in clinical research or practice."

Exercise

Physical Therapy Interventions for Patients With Osteoarthritis of the Knee: An Overview of Systematic Reviews. (Phys Ther. 2007) "There is high-quality evidence that exercise and weight reduction reduce pain and improve physical function in patients with osteoarthritis of the knee. There is moderate-quality evidence that acupuncture, transcutaneous electrical nerve stimulation, and low-level laser therapy reduce pain and that psychoeducational interventions improve psychological outcomes. For other interventions and outcomes, the quality of evidence is low or there is no evidence from systematic reviews."

Hydrotherapy Versus Conventional Land-Based Exercise for the Management of Patients With Osteoarthritis of the Knee: A Randomized Clinical Trial. (Phys Ther. 2007)

Aquatic exercise for the treatment of knee and hip osteoarthritis. (Cochrane Database Syst Rev. 2007) "AUTHORS' CONCLUSIONS: Aquatic exercise appears to have some beneficial short-term effects for patients with hip and/or knee OA while no long-term effects have been documented. Based on this, one may consider using aquatic exercise as the first part of a longer exercise programme for osteoarthritis patients. The controlled and randomised studies in this area are still too few to give further recommendations on how to apply the therapy, and studies of clearly defined patient groups with long-term outcomes are needed to decide on the further use of this therapy in the treatment of osteoarthritis."

Long-Term effectiveness of exercise therapy in patients with osteoarthritis of the hip or knee: A systematic review. (Arthritis Rheum. 2007)

Vigorous Physical Activity May Improve Knee Joint Structures

Tai chi for osteoarthritis: a systematic review. (Clin Rheumatol. 2007)

The Effects of Strength Training among Persons with Hand Osteoarthritis: a Two-Year Follow-up Study. (J Hand Ther. 2007) "Results suggest strength training safely increases dynamic and static grip strength and reduces pain in older persons with hand OA."

Short-term efficacy of physical interventions in osteoarthritic knee pain. A systematic review and meta-analysis of randomised placebo-controlled trials. (BMC Musculoskelet Disord. 2007)

Physical activity for osteoarthritis management: A randomized controlled clinical trial evaluating hydrotherapy or Tai Chi classes. (Arthritis Rheum. 2007) "CONCLUSION: Access to either hydrotherapy or Tai Chi classes can provide large and sustained improvements in physical function for many older, sedentary individuals with chronic hip or knee OA."

Effects of a Sun-style Tai Chi Exercise on Arthritic Symptoms, Motivation and the Performance of Health Behaviors in Women with Osteoarthritis. (Taehan Kanho Hakhoe Chi. 2007)

Effects of aquatic exercise on flexibility, strength and aerobic fitness in adults with osteoarthritis of the hip or knee. (J Adv Nurs. 2007)

General Information

Evidence of Benefit Lacking for Many Common Ways of Treating Osteoarthritis of the Knee "The review found that glucosamine and chondroitin, over-the-counter dietary supplement ingredients that are used widely because of their purported benefits to relieve knee pain caused by osteoarthritis and improve physical functioning, appear to be no more effective than placebos. … The review, which was requested and funded by HHS' Centers for Disease Control and Prevention, also failed to find convincing evidence of benefit from arthroscopic surgery to clean the knee joint with or without removal of debris and loose cartilage."

Guidelines

New EULAR Guidelines for Treating Hand Osteoarthritis "The EULAR task force issued 11 key evidence-based recommendations for treatment of hand osteoarthritis (OA), which are published in the October 17 Online First issue of the Annals of the Rheumatic Diseases. The authors note that hand OA is a commonly undetected condition, but one that causes significant disability."

Chondroitin and glucosamine sulfate in combination decrease the pro-resorptive properties of human osteoarthritis subchondral bone osteoblasts: a basic science study (Arthritis Research & Therapy 2007)

Total joint replacement after glucosamine sulphate treatment in knee osteoarthritis: results of a mean 8-year observation of patients from two previous 3-year, randomised, placebo-controlled trials. (Osteoarthritis Cartilage. 2007)

Glucosamine and chondroitin sulfate as therapeutic agents for knee and hip osteoarthritis. (Drugs Aging. 2007)

Glucosamine for pain in osteoarthritis: Why do trial results differ? (Arthritis Rheum. 2007) "CONCLUSION: Heterogeneity among trials of glucosamine is larger than would be expected by chance. Glucosamine hydrochloride is not effective. Among trials with industry involvement, effect sizes were consistently higher. Potential explanations include different glucosamine preparations, inadequate allocation concealment, and industry bias."

Oral Glucosamine in Doses Used to Treat Osteoarthritis Worsens Insulin Resistance. (Am J Med Sci. 2007)

Glucosamine/chondroitin combined with exercise for the treatment of knee osteoarthritis: a preliminary study. (Osteoarthritis Cartilage. 2007) "CONCLUSIONS: The GH/CS group was not superior to the placebo group in function, pain, or mobility after both phases of the intervention (pill only and pill plus exercise)."

Meta-analysis: Acupuncture for Osteoarthritis of the Knee (Annals 2007) "Conclusions: Sham-controlled trials show clinically irrelevant short-term benefits of acupuncture for treating knee osteoarthritis. Waiting list–controlled trials suggest clinically relevant benefits, some of which may be due to placebo or expectation effects."

Meta-analysis: Chondroitin for Osteoarthritis of the Knee or Hip (Annals 2007) "Conclusions: Large-scale, methodologically sound trials indicate that the symptomatic benefit of chondroitin is minimal or nonexistent. Use of chondroitin in routine clinical practice should therefore be discouraged."

Current role of glucosamine in the treatment of osteoarthritis. (Rheumatology (Oxford). 2007) "The most compelling evidence of a potential for inhibiting the progression of OA is also obtain with GS."

Predictors of symptomatic response to glucosamine in knee osteoarthritis: an exploratory study. (r J Sports Med. 2007) "CONCLUSIONS: Although glucosamine significantly improved symptoms, most of the variance in outcome at 12 weeks was unexplained by the predictors evaluated. However glucosamine may be more effective at improving symptoms in knee OA patients with a lower BMI, PFJ ostophytes and lower functional self-efficacy."

Effect of chondroitin sulfate in symptomatic knee osteoarthritis: A multicenter, randomized, double blind, placebo-controlled study. (Ann Rheum Dis. 2007) "CONCLUSION: This study failed to demonstrate an efficacy of CS on the two primary criteria considered together, although CS was slightly more effective than placebo on pain, OMERACT-OARSI response rate, investigator's assessment and quality of life."
