PediatricsDiagnosis, Imaging, and ScreeningNEWS:Half kids labeled bipolar may be misdiagnosed "Symptoms include extreme mood swings and disruptive behavior. In children, extreme irritability is sometimes the main symptom. … Dr. David Fassler, a University of Vermont psychiatry professor, said research suggests that close to half of children thought to be bipolar may be misdiagnosed. He said parents should get a second opinion if they have concerns about a diagnosis or proposed treatment. "Bipolar disorder is not always easy to recognize in children and adolescents. There's considerable overlap with other conditions, including ADHD, conduct disorder, anxiety disorders and depression," said Fassler, who was not involved in Olfson's study." (Scandinavian Journal of Infectious Diseases 2007) [Bipolar paediatric disorder early diagnosis] (Rev Med Suisse. 2007) "Irritability, impulsiveness, hyperactivity, intolerance to frustration and sleep disorders are the most frequent prodroms. If they present a clinic of suicidality, toxic abuse (cannabis, alcohol), or even psychotic symptoms, in the context of significant family history, the child psychiatrist should not doubt to diagnose Bipolar Disorder and start the corresponding treatment." (Clin Exp Rheumatol. 2007) "CONCLUSION:FMS in young children of 10 years old and younger is frequently under-recognized. As compared with the older group, stiffness, subjective joint swelling, abdominal pain, initial presentation on wheelchair and a higher mean count of tender points at diagnosis were significantly more common in the younger age group. However, the type of medications used and outcome were similar in both groups. Prospective studies with large patient population are needed to clarify these findings." Other than fever and rash, only nausea/vomiting (73%) and headache (61%) were present in more than 50% of cases. The authors calculated that only 58% of subjects had "classic" symptoms of fever, rash, and headache. Fever, rash, and tick exposure were present in only 45% of cases. Median leukocyte counts were not elevated (median, 9500/microliter), but 59% of patients had platelet counts < 150,000/mm3. Liver function studies and clotting labs were normal in the majority of patients." Underdiagnosis of Hypertension in Children and Adolescents (JAMA. 2007) |
